By submitting this form, you confirm that you are a licensed clinician in good standing with the applicable regulatory body in your jurisdiction, and that you are applying on behalf of your own practice or your employer. Vesalius Longevity Labs will verify your credentials and license status before activating your provider account. Provider accounts are available only to licensed physicians (MD/DO), nurse practitioners (NP), physician assistants (PA), naturopathic doctors (ND), and other licensed prescribers in jurisdictions where peptide therapeutics may be legally prescribed. Vesalius does not accept applications from unlicensed individuals or for the purpose of resale to end consumers. Products ordered through the Vesalius platform must be dispensed pursuant to a valid patient-specific prescription in accordance with applicable law.
